Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Other violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.


Any establishment receiving 40 red points or any red point item repeated within an 18-month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

Denny's #7541, Centralia, Harrison Avenue, 0 points. No significant findings were noted outside of a small change to the children's menu and minor adjustments to coolers.

Fischer's Market, Randle, 0 points. Notes during this inspection were very positive regarding proper use of hold temperatures, having all required safety equipment and all food worker cards posted and current.

Gene & Barbs', Randle, 0 points. No violations noted.

Hillcrest Food Mart, Chehalis, 0 points. Everything in compliance, no violations noted.

I-5 Grocery, Chehalis, N. Hamilton Road, 0 points. No violations noted.

Judy's Country Kitchen, Centralia, Galvin Street, 0 points. No violations noted.

Pe Ell Pub, Pe Ell, N. Main Street, 30 Red. The bulk of the red violations were due to improper handling of food products during assembly and lack of gloves for staff. The violation was corrected during the time of the inspection. Additional violation was noted for one expired food worker card.

Pioneer Bar & Grill, Mossy Rock, State Street, 30 Red, 35 Total points. Here violations were varied with no paper towels for hand washing station, improper temperatures for cold holding and improper storage of different types of food. There was also a lack of a digital thermometer at time of inspection. There is also a requirement to fix a direct draining sink.

Riverside Bistro, Chehalis, Airport Road, 20 red points. Violations here are attributed to staff and knowledge of food safety procedures from basic knowledge to certifications from unapproved websites. There were also violations with re-heating food, improper heating practices (lack of monitoring on steam table) and kitchen preparation temperatures.

Rumors, Chehalis, Pacific Avenue, 0 points. No violations noted. Everything in order.

Shell Station, Napavine, Rush Road, 0 points. No violations noted.

Sweet Shots, Chehalis, State Avenue, 10 Red points. Milk was found at wrong temperature - was moved to correct violation.

Spiffy's Dine-In Drive-In, Morton, Westlake, 0 points. No violations noted.

Taqueria La Mexicana (Mobile vendor), Centralia, Harrison Avenue, 10 Red points. With no water available and refrigeration not working properly - the establishment was advised to stop food preparation until water was available.

Taco Del Mar, Chehalis, Louisiana Avenue, 0 points. No violations noted.

Temporary Permit Inspections

Reliable Enterprises, Chehalis, N. National, 20 Red points. No soap at hand washing station - was corrected. Cold food over temperature, request for adjustments to refrigerators.
