who have died in the past year. We invite family members to place a symbol of remembrance on our "Memory Tree" that is located outside our offices on Pacific (across from McDonalds) in Long Beach.

The "Memory Tree" lighting is set for Wednesday, December 4th at 4:30 p.m. with a short ceremony. Staff members will hang Oyster Shells with the first name of all the hospice patients who have died in the past year. We encourage community members to join us at that time, or to take the opportunity to privately place a remembrance on our tree anytime during December.

We also invite everyone to stop by on December 4th anytime between 4-6 p.m. to meet our staff, share some holiday cheer and learn how our excellent teams might assist your family member to heal at home.

Our Home Health Team provides Registered Nurse Coordination of care that could include physical, occupational and speech therapies, nursing aides and Social work support. Our nurses provide support for surgical recovery, diabetes management and other chronic medical conditions such as CHF, COPD and cardiac diseases.

Our Hospice Team members strive to help patients and families live in waysthat honor their values and goals. Working with physician, patient and family our team creates a safe and compassionate home environment that includes effective pain management in which to enjoy the last days of life.
